Community Alternatives Program

For Disabled Adults (CAP/DA)

CAP/DA provides a varied and person centered service array designed to enable adults (age 18 and older) who meet both Medicaid and CAP/DA eligibility, as well as nursing facility level of care, to remain in their private residence. The following services are offered under our program:

• Personal Emergency Response system
• Home delivered meals
• Oral nutritional supplements
• Incontinence products
• Medication boxes
• Home accessibility and adaptations
• Participant goods and services
• Community Transition services
• Training, education and consultative services
• Assistive technology

For Disabled Adults - Choice (CAP/DA Choice)

CAP Choice is a consumer directed option under CAP/DA and offers all the same services above, but has a few additional services specifically designed to help participants direct their own care including:
• Care Advisement
• Financial Management
• Personal Assistant Services

The individual needing services or someone acting on the individual's behalf can call the Adult Services Line at 704-920-1400 Press Option 7, then Option 2 to request assistance. Requests for assistance can also be made in person.
