Certain residents in adult care homes have extensive needs for personal care assistance and qualify for additional support by Medicaid for this purpose. The function of case management is to verify the need for this additional assistance and to assure that the resident's needs for enhanced personal care, as well as needs for other related health and social services are being met.
An individual must be a resident in an adult care home and be eligible for Medicaid. In addition, the resident must meet Medicaid criteria as a heavy care resident, requiring extensive or total assistance in ambulation/locomotion and/or toileting and/or feeding.
The individual needing services or someone acting on the individual's behalf can call the Adult Services line at 704-920-1400 to request assistance. Requests for assistance can also be made in person.
